Sad Denny. This goes deeper as just corruption. It's even worse over here. The autocracy puts inexperienced/unqualified people into positions that needs highly skilled professionals.  

When I started working in the industry in the mid 90's, we had someone from the engineers representing the client on site permanently.  These days it's rare and only bi-weekly visits seems to be the norm. The clients does not want to pay for the oversight anymore...the whole process has been watered down.

We're supposedly building under strict rules......the mind boggles as to how it's allowed to happen. There's also been 2 deaths from recent building accidents. 

Read a report that there's roughly 13,000 building applications in Cape Town

Rules are only good if you have competent and honorable people doing oversight...money removes many obstacles... That looks like deliberate substandard work or gross incomplete.  Concrete work is difficult in some regards, but if you folow the guidelines, it's not possible to get it that wrong without knowing about it. Those kind of failures looks like deliberate substandard work...and to get away with it, you need to fake test results.
