That is a classical insult in Sicily.

Unaware people may also confuse the thumb up gesture for an encouraging, friendly sign as it is understood elsewhere.

In Sicily, that is an insult meaning what it means above.

Reminds me of misunderstandings between languages and cultures can end up in fisticuffs.

A couple of decades ago, an Afrikaans friend of mine was visiting either Italy or Greece (cannot remember which) and he had another Afrikaans buddy with him, his name was Isak De Kok (known as Sakkie). They were having a few glasses of wine in a bar or tavern of sorts and became chatty and friendly with the locals, they eventually got round to introductions and handshakes and when Sakkie was introduced to one and announced his name "Sakkie De Kok" it almost broke into a bar room brawl....hahahaha.
